then the institution is at fault.
Part of the reason we have institutions is to oversee and correct the mistakes of people who may not know their job or may be unfit for it.
Well managed, institutions can retain talent and redirect incompetence out of the professions. That's their function in terms of staff.
Have we forgotten this much about basic social science?
